I am NOT prone to Fainting or Vertigo.

I have NOT consumed any Anticoagulants within the last 24 hours (such as but not limited to, Aspirin or Ibuprofen).

I fully understand that variations may occur between the image provided or chosen and the actual tattoo and that tattoos will fade over time and with exposure to light.

I ACKNOWLEDGE AND FULLY UNDERSTAND that by using Nuf Ced Ink Tattoo Studio you will be engaging in activities that involve but are not limited to serious injury, scarring, disfigurement, illness, permanent disability or death.

Client hereby assumes all risks and freely accepts full personal responsibility for all potential injuries associated with the services rendered by Nuf Ced Ink Tattoo Studio.

In consideration of being provided services, the Client does for him/herself, his/her heirsd, executors and assigns, hereby release, waive and discharge Nuf Ced Ink Tattoo Studios, it's predecessors, succesors, parents,affiliates, officers, representatives, insurers and employees from any and all actions, claims, demands, accidents, injuries, death, damages,whether known or unknown arising from the use of the services rendered bu Nuf Ced Ink Tattoo Studio.
